What Is the Rural Health Transformation Program?

The Rural Health Transformation Program (RHTP) is the largest federal investment in rural healthcare in U.S. history. Created by Section 71401 of the One Big Beautiful Bill Act (Public Law 119-21), RHTP provides $50 billion over 5 years to strengthen healthcare delivery in rural America.

How It Works

CMS distributes $10 billion per year to all 50 states through cooperative agreements. Each state designates a lead agency, develops a Rural Health Transformation Plan, and distributes funds to eligible subrecipient organizations — hospitals, clinics, behavioral health providers, EMS agencies, and community health organizations.

How RHTP Funding Works

RHTP distributes $10 billion per year across all 50 states. Half is distributed equally; the other half is variable based on rural population, health outcomes, and need.

Spending Caps

  • 10% — Administrative costs
  • 10% — Technology Innovation Catalyst Fund
  • 15% — Provider payments
  • 20% — Capital expenditures
  • 5% — EHR system replacement

Key Financial Rules

  • No state matching funds required
